Disciplinary Reports
When an inmate violates institutional administrative rules or inmate Rules of Conduct, a corrections officer may write a disciplinary report. Depending on facility policy, some cases may warrant a verbal warning or written warning before the officer files a written disciplinary report. According to the California Board of State and Community Corrections (n.d.), disciplinary reports are used to document inmate behavior, justify administration segregation, provide information for classification, and as a cause of action for disciplinary hearings (p. 9). A sample disciplinary report is provided in Figure 4.1.
Disciplinary reports may document an inmate who possesses contraband (phone, drugs, etc.), acts violently, or assaults an inmate or officer. Olson (2016) observed that prison overcrowding, high inmate-to-staff ratios, and extensive rules often contribute to violations that require inmate discipline (p. 760). In the News 4.2 illustrates the various institutional causes for disciplinary action and the types of infractions that often require a disciplinary report.
When writing a disciplinary report, officers should follow the guidelines provided earlier in this chapter on writing an incident narrative. Some agencies may use a separate inmate disciplinary report, such as the one illustrated in Figure 4.2. But not all agencies do this. Since the disciplinary report closely mirrors an incident report, some agencies use the incident report form to document inmate misconduct. As with an incident report, the disciplinary report must be detailed and well written in all respects.
